Forms

The trial Forms page shows forms connected to scheduled activities. Each placement includes the form version, activity, visit, order, and requirement. Open a row to view the reusable form. Removing a placement does not archive the form.

Generate forms

From Forms, select Generate, choose a trial, then choose one or more activities. From Build > Forms, the trial is already selected. Each activity shows its connected visits and related forms. Activities with complete coverage cannot be selected.

Generation prepares one review for all selected activities. It checks existing forms and includes only visits without a form placement.

On Build > Forms, select Connect. Choose an activity, visit, and form. Saving creates a placement without changing the reusable form. The Forms field is unavailable when no forms exist.

Each generated result can reuse a form, create a draft, or explain why no form is needed. Review every result before saving. If the schedule changes, generate a new review.

CDASH mappings require supporting evidence. Generation does not guarantee CDASH or vendor compatibility.
